When a youth is close to meeting their goals for placement, the Primary Counselor will update their post-placement plan, including a rules/expectation list. The post-placement plan will be used as a guide.
Transition into the post-placement site should include visits of increasing length with family counseling appointments during the visits, if needed. If a foster home is determined as the post-placement site, attempts can be made to set up family counseling in coordination with visits to the foster home.
Youth may come to Port as a step down program right before they reintegrate into their home community.
